Implementing Behavioral Advisory Flag System to Mitigate Workplace Violence

To combat rising workplace violence in the Emergency Room, I led the deployment of a Behavioral Advisory Flag (BAF) system across a Franciscan Healthcare System spanning two states. By integrating electronic health record (EHR) tracking with physical magnetic cue cards, the project established a critical buddy system for staff and volunteers interacting with high-risk patients. Delivered in just six months, the solution successfully overcame hurdles that had stalled similar initiatives globally for over two years, resulting in a safer environment and mitigating significant nurse turnover and injury costs.

The Challenge

Healthcare workers and bystanders in the Emergency Room face significant risks of verbal and physical abuse from patients impaired by alcohol, drugs, or mental health conditions. Workplace violence in healthcare is a costly epidemic, with the American Hospital Association estimating annual financial costs at over $18 billion, and the turnover cost for a single bedside RN reaching upwards of $61,110. Prior to this initiative, extensive research and partnerships with multiple Healthcare Systems across the United States and Europe revealed a common pain point: the majority of these organizations had unsuccessfully attempted to implement a reliable violence tracking system for over two years. The challenge required balancing staff safety, patient privacy, and operational efficiency without causing workflow bottlenecks or legal exposure.

Architectural Approach

Anticipating the roadblocks that hindered other health systems, I led a proactive, multidisciplinary approach involving Risk Directors, the C-suite, and the Legal team. Within six months, a dual-layer Behavioral Advisory Flag system was successfully rolled out across a bi-state Franciscan Healthcare System. The solution seamlessly integrated behavioral flags within the electronic patient records system alongside a physical, color-coded magnetic cue card system placed on patient door frames. This visual alert ensures that anyone—from clinical staff to a hospital volunteer delivering flowers—immediately recognizes the need to use a buddy system before entering the room. This comprehensive framework directly addresses safety pain points, protects bystanders, and provides a scalable model for violence prevention in acute care settings.
